Vethara's Unfinished Index
Vethara was the head archivist of the Sorrenmere Library, a vast collection that burned under circumstances scholars still dispute. She died before completing her life's work, a grand index of every creature ever catalogued in the stacks, and her obsession bled into the pages. The book that survives is not the finished index but something worse: a record that keeps updating itself, filling its margins with the last thoughts of the newly dead in Vethara's own restless hand.
Mechanics
A ghost-annotated tome that lets you interrogate the residual knowledge of a slain creature and confound enemies with cursed script.
The Margin Whispers
When you reduce a creature to 0 hit points, you may use your reaction to absorb a fragment of its fading awareness. Choose one of the following: the creature's resistances and immunities, its saving throw proficiencies, or whether it has any abilities that recharge on a die roll. The book records this information in cramped phantom script visible only to you, and you retain it until you use this ability again on a different creature.
Blighted Annotation
As an action, you open the book to a page of writhing cursed script and force a creature you can see within 30 feet to confront it. The target must succeed on a DC 15 Wisdom saving throw or become frightened of you until the end of its next turn. While frightened in this way, the creature also has disadvantage on Intelligence and Wisdom saving throws. Once you use this ability, you cannot use it again until the next dawn.
DM Notes
The reaction trigger on The Margin Whispers is powerful because it costs no resource and provides genuine tactical intelligence. Balance consideration: the information is retrospective, arriving after the creature is already dead, which means its value lies in pattern recognition across a dungeon rather than in-combat advantage. DMs should ensure there are living creatures of the same type nearby for the knowledge to matter. Blighted Annotation pairs well with the frightened condition's movement restriction, making it useful for casters who want to force melee creatures to waste turns. The DC 15 Wisdom save is appropriate for rare tier but will be less reliable against high-Wisdom enemies such as undead commanders or fiends, which fits the haunted-library tone of the item. Consider placing this in the possession of a ghost or lich archivist to give it narrative weight.
